Transition and Sustaining Referrals
We currently have two openings for HSS Transition or Sustaining referrals. DHS is running between 6-8 weeks on approvals once the paperwork is submitted.

Eligible Members:
A member is eligible for housing stabilization services if they meet all of the following needs-based criteria:
Be on Medical Assistance (MA)
Be 18 years old or older
Have a documented disability or disabling condition,
